You have the right to see your health records. New York State Law requires all healthcare providers to allow patients to look at their health records. You can ask to see your own records or your child's records.
Make your request in writing, include the date of your request, and keep a copy for yourself. Your healthcare provider has 10 days to show you the records.
If your healthcare provider will not let you see your records, you can file a form with the New York State Department of Health. State Form 1989 has instructions that describe your rights and what records you can see (the healthcare provider does not have to show you everything). The instructions tell you how to appeal if your healthcare provider does not cooperate.
