An HIV Special Needs Plan (SNP) is for someone who is:

  • eligible for Medicaid and
  • living with HIV/AIDS, homeless, or transgender. 

A SNP includes the same services as other Medicaid plans. It also provides special services for people with HIV/AIDS.

There are many SNPs to choose from. If you sign up for a SNP, you get:

  • access to an HIV specialist primary care provider,
  • HIV care coordination services,
  • information on medications and side effects, 
  • support to stick with your treatment, and
  • other specialty services.
