An HIV Special Needs Plan (SNP) is for someone who is:
- eligible for Medicaid and
- living with HIV/AIDS, homeless, or transgender.
A SNP includes the same services as other Medicaid plans. It also provides special services for people with HIV/AIDS.
There are many SNPs to choose from. If you sign up for a SNP, you get:
- access to an HIV specialist primary care provider,
- HIV care coordination services,
- information on medications and side effects,
- support to stick with your treatment, and
- other specialty services.
